Do I need a referral?

No. Both services are available directly without a referral.

Is this covered by insurance or Medicare?

No. Both services are cash-pay only. They are not billed to any insurance plan or to Medicare. Testing that your primary care physician orders on your behalf may be covered by your insurance — that is between you and your physician.

Is this available outside of Ohio?

Currently, services are available to residents of Ohio. We plan to expand to additional states based on demand.

Is this a replacement for my primary care physician or neurologist?

No. This practice provides advisory and wellness services. If clinical concerns are identified, you will be advised to follow up with your treating physician or an appropriate specialist.

What if you find something that needs medical attention?

If any assessment reveals a finding requiring clinical evaluation or urgent attention, Dr. Hashmi contacts you directly and recommends appropriate follow-up. Your safety is the first priority, always.

How is this different from other brain health or longevity programs?

Most brain health programs are run by health coaches or wellness practitioners applying general lifestyle advice. This practice is led by a board-certified neurologist with fellowship training in neuroimaging — which means your risk assessment includes the ability to evaluate your brain’s structure at a level that non-neurologists cannot. Every recommendation in your plan traces to peer-reviewed research. There are no proprietary supplements, no unproven therapies, and no upsells. If the evidence doesn’t support it, it’s not in your program.

The Cognitive Resilience Program

How much time does this require from me?

You’ll have a personalized initial consultation, a few hours over the first month obtaining baseline testing, and then regular video consultations over the course of the year. Between visits, you implement your plan in your daily life — the program is designed to integrate with how you already live, not to overhaul everything at once.

What kind of testing is involved?

The program includes a comprehensive battery of assessments — bloodwork, cognitive testing, and optional advanced evaluations. Dr. Hashmi provides specific guidance on what testing is recommended for you and how to obtain it. Testing costs are paid separately by you, directly to the testing facility.

How do the video consultations work?

All consultations are conducted via a secure, HIPAA-compliant video practice. You’ll receive a link before each scheduled visit. All you need is a computer or phone with a camera, a stable internet connection, and a quiet, private space.

Can I do this if I’m already working with a longevity physician or functional medicine practitioner?

Yes. This program focuses specifically on the brain — which is typically the gap in an existing longevity or optimization stack. Dr. Hashmi can complement your existing care. All program documents are yours, and you’re free to share them with any provider.

What if I have a family history of Alzheimer’s or carry the APOE e4 gene?

This program is well-suited for people in your situation. The research suggests that addressing modifiable risk factors may be particularly important for individuals at elevated genetic risk. Dr. Hashmi can discuss how the program adapts to your specific circumstances during the discovery call.

I had a concussion — or multiple concussions — years ago. Is this program relevant for me?

It can be. A history of head injury is itself a risk factor for long-term cognitive decline, and the program’s assessment and optimization framework can be adapted for patients with prior concussion or TBI exposure. Dr. Hashmi can discuss whether the program is appropriate for your situation during the discovery call.

What if I move out of Ohio during the program?

This program is currently available to Ohio residents. If you relocate during the program, we’ll discuss options at that time based on where you’re moving. In some cases, we may be able to continue; in others, we’ll help you transition your plan to a local provider.

What happens after 12 months?

At the end of the program, you receive a comprehensive annual report comparing your baseline to your current data across every metric. You then have the option to continue into a maintenance year or to graduate with your report and plan as your ongoing reference.

Does this program guarantee I won’t develop dementia?

No. No program, physician, or intervention can guarantee that. What this program does is systematically identify and address the modifiable risk factors that research has linked to cognitive decline. The evidence for multi-domain risk factor intervention is strong and growing — but it is risk reduction, not prevention. We will always be honest with you about what the science shows and what it doesn’t.

What types of imaging do you review?

Brain MRI, spine MRI (cervical, thoracic, lumbar), brain CT studies, and CT spine (cervical, thoracic, lumbar) studies. If you’re unsure whether your study qualifies, contact us before submitting.

How do I get my images?

We provide step-by-step instructions for obtaining your DICOM images from any imaging facility.

What if the images aren’t usable?

If your images are insufficient quality or the wrong study type for meaningful review, you receive a full refund.

Is this a diagnosis?

No. NeuroSignal Review is an advisory service. The report provides expert interpretation and educational guidance. It does not constitute a clinical diagnosis and does not establish a treatment relationship.

Do I need cognitive symptoms to join the program?

No. Many participants enroll proactively to establish a baseline or because of family history. Whether you’re noticing subtle changes or simply want a data-informed roadmap for long-term brain wellness, the program is built for both prevention and early detection.

What if I don’t have a recent MRI?

You can still participate. If you have a recent MRI, we incorporate volumetric analysis. If not, we tailor the program based on available data and discuss whether obtaining an MRI would add meaningful value. The program does not require one to begin.

Is qEEG required?

No. qEEG brain mapping is optional and included only when available or clinically helpful. It offers additional insight into electrical activity patterns but is not necessary for a complete evaluation.

How long does the program last, and what does follow-up look like?

The program is structured over 14 months, with periodic telehealth sessions to review progress, interpret new data, and refine your personalized plan. You receive consistent guidance rather than a one-time consultation.
