The Cognitive Resilience Program is a 12-month, physician-led engagement that begins with a level of brain health assessment rarely available outside of a research setting — and translates what we find into a personalized, evidence-based plan that you follow with a dedicated neurologist guiding every step.

Exercise has the strongest evidence of any single behavior for supporting long-term brain health. This program assesses where you are and builds targets based on the best available evidence.

What’s happening in your blood vessels is happening in your brain. Blood pressure, blood sugar, cholesterol, and body composition all matter — and they’re all measurable and modifiable.

Sleep

Sleep is when your brain performs critical maintenance. Poor sleep quality is associated with accelerated brain aging — and it’s one of the most actionable risk factors addressed in this program.

NUTRITION

Specific dietary patterns have been associated with slower rates of cognitive decline in large studies. This program goes beyond general advice to assess your actual biochemical status and build a targeted nutritional strategy.

COGNITIVE AND SOCIAL ENGAGEMENT

Social isolation, untreated hearing loss, depression, and loss of purpose are among the strongest risk factors for cognitive decline — and among the most overlooked. This program addresses all of them.

What you receive

The Lancet Commission on Dementia Prevention identified 14 modifiable risk factors that account for up to 45% of dementia cases worldwide. 

The FINGER trial — the first large-scale randomized controlled trial of multi-domain lifestyle intervention — demonstrated that simultaneously addressing these risk factors led to significant cognitive benefits compared to standard health advice. 

The U.S. POINTER trial is now testing whether these results replicate in a diverse American population. 

This program takes that research and makes it personal. Not a pamphlet. Not a webinar. A physician who knows your data, your risk factors, and your life — and works with you for a full year to address every modifiable factor in your profile.

Comprehensive Assessment

You complete a detailed intake. You undergo a battery of advanced assessments — bloodwork, cognitive testing, and optional brain imaging with quantitative analysis. Dr. Hashmi reviews everything personally and prepares two personalized documents: your risk profile and your optimization plan. Then you meet for a comprehensive video consultation where every finding is explained and your plan is built — together.

Active
Optimization

Over the following months, you meet with Dr. Hashmi monthly via video for focused consultations that work through each domain systematically. You receive written progress notes after every visit. All reports and documents are yours to share with any physician you choose.

Program
Reassessment

At 12 months, you repeat your assessments. Dr. Hashmi  prepares a comprehensive annual report comparing where you started to where you are now — across every metric. You see, in data, where you stand compared to where you started. From there, you can continue into an optional maintenance year with periodic consultations, or graduate with your report and plan in hand.

If you are experiencing progressive memory loss, confusion, personality changes, or difficulty with daily activities, you need a clinical neurological evaluation — not this program.
This program is not medical treatment. It is a structured, evidence-based wellness and advisory engagement. We are happy to help you find appropriate care.
No medications are prescribed through this program. It does not replace your primary care physician or specialists.

The Cognitive Resilience Program — Year 1 

$7,500 — paid as $2,500 at enrollment plus 8 monthly payments of $625. 

$6,000 — paid as $2,000 at enrollment plus 8 monthly payments of $500. Available for the first 10 enrollees only.

$3,000 per year — quarterly physician consultations, annual reassessment, and annual report. 

Testing costs (bloodwork, cognitive assessment, imaging) are paid separately by the patient directly to the testing facility. Dr. Hashmi provides specific guidance on what to obtain and where. 

The Cognitive Resilience Program — Year 1

$6,000 — paid as $2,000 at enrollment plus 8 monthly payments of $500. Available for the first 10 enrollees only.

Maintenance Year (Optional, Year 2+) 

$3,000 per year — quarterly physician consultations, annual reassessment, and annual report.
